    24 hour Advice Line Michael Sobell Hospice

    Offers support and advice on palliative care issues to GPs, Care Homes, District Nurses and hospital doctors.

    It is also an Advice Line for patients and families who live in Hillingdon and need advice on any aspect of palliative care. 

    The Advice Line is answered by hospice nurses in the Inpatient Unit and queries are escalated to the on-call palliative medical team if needed.

    Pembridge Hospice provides palliative care services and advice to the residents of Brent, Central London, West London and Hammermith and Fulham.

    This is a 24 hour, 7 days a week telephone advice line.

    This is a 24 hour 7 days a week helpline providing advice for patients, families and professionals, hosted by St Luke's Hospice for residents of Harrow and North Brent.

    Urgent rapid response visits can be made to patients registered with a Harrow GP

     

    The team is made up of clinical nurse specialists and health care assistants who can support patients with complex needs at home in the last weeks and months of their life (with any diagnosis). 

    The service is open 24 hours a day, 365 days a year, and can offer advice.

     Urgent night visits  are available to patients already referred to the service.

    If you have already been referred to this service, you will have been given a direct contact number.

    Speak to your GP or district nurse to be referred to the service.

    Bereavement Advice Centre

    Bereavement Advice Centre provides practical help when you need it most. They support and advise people on what they need to do after a death.

    Trained volunteers to support adults and children in the London Boroughs of Harrow and Hillingdon.

    Offer visits, drop in sessions and school support. 

    Self referrals accepted as well as referrals from healthcare professionals, the local authority, IAPT/ CAMHS. 

     

    Bereavement Counselling Service Westminster

    Bereavement Counselling Service offers free bereavement counselling to any adult living in, or registered with a GP in the borough Westminster. Counselling can be either during the day or the evening, and is provided by trained and supervised volunteer counsellors on a regular weekly basis.

    Bereavement Support at Royal Trinity Hospice

    The Bereavement team supports family members, friends, children and carers through the emotional impact of grief. The support is available to anyone who has experienced the death of a loved one who was cared for by the Hospice. 

    The Bereavement team offer support specifically for children and guidance to parents on how to explain death and grief to children.

    Bereavement Support at St. Luke's Hospice

    The Bereavement Support Team offers confidential, individual support to help cope with the loss of family or loved ones. The support is available to anyone who has experienced the death or affected by the death of a loved one  who was cared for by St Lukes. 

    Bereft Bereavement Support was set up in 1999 to fill the need for specialist bereavement counselling and support for the adult residents (the over 16s) of the London Borough of Ealing.

    Bereavement counselling can help you explore your feelings and come to terms with your grief by talking one to one with a trained counsellor. During these sessions you can talk about how you feel and your counsellor will listen and comment, rather than giving you advice.

    The Child and Adolescent Bereavement Service (CABS) run by Harlington Hospice provides support for children and young people aged between the ages of 4-17 and their families/ carers who live in Hillingdon and who have been bereaved or who have a loved one with a life-limiting diagnosis. 

    Referrals are accepted via email from schools, healthcare professionals, parents/ carers and young people over 16. 

    CABS offers 1:1 counselling, group sessions and family sessions.

    The Child Bereavement Service is a free service provided by a specialist social work team with a long track record of providing practical and emotional support to individuals and families before and after a bereavement.

    Child Death Helpline is operated by a dedicated team of volunteers who are all bereaved parents.     CNWL Talking Therapies Service provides psychological therapies. Self-referral accepted via the website for adults who are registered with a Brent, Harrow or Hillingdon GP.

    Support is available for a range of difficulties, such as low mood or depression, anxiety, and stress, phobias, physical health anxiety, PTSD and Insomnia.

    Community Action on Dementia Brent

    The Dementia Hub Project  in Brent offers tailored support to people living with dementia and their families. The team can provide advice about benefits, the social care system and how to access wellbeing support such as Dementia Cafés.

    Community Palliative Care Team - based at Meadow House Hospice

    The Community Specialist Palliative Care Nurse Team provides advice and visiting to palliative care patients in Ealing and Hounslow. The team are based at Meadow House Hospice.

    The team will visit patients with progressive life limiting illness in their own homes. They provide specialist advice around pain and symptom control and support for patients, their families, and carers during the last stages of illness. They also support patients wishing to die at home through coordination with GPs and hospital teams.

    The team is comprised of medical consultants, specialist nurses, physiotherapists, occupational therapists, social workers, a bereavement support officer and a patient and carer advocacy worker

    Compassionate Hillingdon is a collaborative project that supports people who might need support in Hillingdon. Their work revolves around a commitment to those living with a life limiting illness, older people and those suffering from isolation. They do this by facilitating reciprocal friendships between like minded people who live close to one another across Hillingdon.

    This project is part of the ‘Compassionate Communities’ movement. Compassionate Communities is built on a combined ethos of a Public Health Approach to Community Development, Palliative and End of Life Care.

    There is a legal requirement that a death be registered within five working days. It is important to let the Register Office know if the funeral is to be held within 24 hours. The person registering the death must be able to provide some basic details of the person who has died. Please take the Medical Certificate of Cause of Death to the Register Office. It is also helpful to take the person’s birth and marriage certificates, passport or NHS Medical Card, although they are not essential.

    H4ALL supports Hillingdon residents to better manage long term conditions and brings residents into greater contact with their local community.

    Referral criteria: Adults over the age of 18 who are registered with a Hillingdon GP and who are isolated/ lonely and who have one or more long term condition.

    Halo is a new charity in Hillingdon supporting bereaved children, young people and their families. They hold a monthly drop-in support group in Uxbridge, offer peer support, a bereavement book library, counselling and much more. A referral form is available on their website.

    IAPT stands for improving access to psychological therapies. IAPT offers talking therapies for people over 18 who have a GP in the London Borough of Hammersmith and Fulham.

    Hammersmith and Fulham Talking Therapies service can help you with common problems like stress, anxiety and depression and use different talking therapies to help you feel better.

    Harlington Hospice provides a wide range of palliative care services to the residents of Hillingdon and surrounding areas.

    The Hospice hosts a Wellbeing Service (shared with Michael Sobell Hospice), complementary therapies, counselling, lymphoedema service and the CABS team supporting children and adolescents.

    Meadow House Hospice provides inpatient palliative care services to the residents of Ealing.

    The hospice has a 15 bedded inpatient unit supported by a team of dedicated staff providing 24/7 specialist support to patients and their family members. Patients with a progressive life limiting illness can be admitted for symptom management or end of life care.

    Patients have a safe, dignified space of their own and the flexibility to spend time alone or in company, surrounded by their possessions and the people they care about. The hospice also has an extensive garden area that provides a quiet and reflective environment for both patients and family members.
