    24 hour Advice Line Michael Sobell Hospice

    Offers support and advice on palliative care issues to GPs, Care Homes, District Nurses and hospital doctors.

    It is also an Advice Line for patients and families who live in Hillingdon and need advice on any aspect of palliative care. 

    The Advice Line is answered by hospice nurses in the Inpatient Unit and queries are escalated to the on-call palliative medical team if needed.

    A

    The Admiral Nurses at Dementia UK provide information, advice and support with any aspect of dementia.

    The specialist dementia nurses offer free and confidential support via the Helpline for you when you need help.

    Adult Social Care provides practical support to people to stay safe and well.  These services are usually provided in people's homes, care homes or in the community. Thre is also support for people who are caring for an adult family member or friend, access to personal care and home care services.

    Age UK provide information and support on financial, legal, health and wellbeing for older people. They support via their advice line, befriending service and day centres.

    Alzheimer’s Society

    The Alzheimer's Society dementia advisors can help people living with dementia and their carers. They can provide practical help and emotional support via the telephone support or the online community connecting those in similar situations.

    Asthma + Lung UK provides help for those living with lung conditions.

    Bereavement Advice Centre

    Bereavement Advice Centre provides practical help when you need it most. They support and advise people on what they need to do after a death.

    Trained volunteers to support adults and children in the London Boroughs of Harrow and Hillingdon.

    Offer visits, drop in sessions and school support. 

    Self referrals accepted as well as referrals from healthcare professionals, the local authority, IAPT/ CAMHS. 

     

    The support group is a way to learn more about living with a lung condition and share your experiences and stories with others.

    British Heart Foundation

    The British Heart Foundation provides information on heart and circulatory diseases.

    Patients and carers can get support through the online support groups and communities, advice and guidance about heart and circulatory conditions and managing risk factors.

    The British Lymphology Society provides a directory of Lymphoedema treatment services.

    The Child and Adolescent Bereavement Service (CABS) run by Harlington Hospice provides support for children and young people aged between the ages of 4-17 and their families/ carers who live in Hillingdon and who have been bereaved or who have a loved one with a life-limiting diagnosis. 

    Referrals are accepted via email from schools, healthcare professionals, parents/ carers and young people over 16. 

    CABS offers 1:1 counselling, group sessions and family sessions.

     

     

    Carers Trust Hillingdon offer support and guidance to unpaid carers of all ages throughout the London Borough of Hillingdon.

    They provide information, advice, training and support and a range of Fact Sheets on their website.

    Child Bereavement UK

    Offer a telephone support service for bereaved young people up to age 25, their families/ carers or individuals or families who have lost a child. 

     

    Child Death Helpline

    Child Death Helpline is operated by a dedicated team of volunteers who are all bereaved parents. The helpline provides support to a parent who has lost a child of any age, under any circumstances, and however recently or long ago, we’re here to help. They also offer support to grandparents, siblings and the extended family, as well as professionals who have been impacted by the death of a child.

    Citizens Advice offers independent and confidential advice over the phone for free.

    CNWL Talking Therapies Service

    CNWL Talking Therapies Service provides psychological therapies. Self-referral accepted via the website for adults who are registered with a Brent, Harrow or Hillingdon GP.

    Support is available for a range of difficulties, such as low mood or depression, anxiety, and stress, phobias, physical health anxiety, PTSD and Insomnia.

    Compassionate Hillingdon is a collaborative project that supports people who might need support in Hillingdon. Their work revolves around a commitment to those living with a life limiting illness, older people and those suffering from isolation. They do this by facilitating reciprocal friendships between like minded people who live close to one another across Hillingdon.

    This project is part of the ‘Compassionate Communities’ movement. Compassionate Communities is built on a combined ethos of a Public Health Approach to Community Development, Palliative and End of Life Care.

    There is a legal requirement that a death be registered within five working days. It is important to let the Register Office know if the funeral is to be held within 24 hours. The person registering the death must be able to provide some basic details of the person who has died. Please take the Medical Certificate of Cause of Death to the Register Office. It is also helpful to take the person’s birth and marriage certificates, passport or NHS Medical Card, although they are not essential.

    Grief Encounter is a national charity offering a free helpline and online support for children and young people wishing to access bereavement support.

    They also offers counselling support in classrooms, online or at their premises in North London. A referral form is available on their website.

    H

    H4ALL supports Hillingdon residents to better manage long term conditions and brings residents into greater contact with their local community.

    Referral criteria: Adults over the age of 18 who are registered with a Hillingdon GP and who are isolated/ lonely and who have one or more long term condition. 

    Services offered: Social prescribing, wellbeing services, counselling and advice.

    Halo is a new charity in Hillingdon supporting bereaved children, young people and their families. They hold a monthly drop-in support group in Uxbridge, offer peer support, a bereavement book library, counselling and much more. A referral form is available on their website.

    Harlington Hospice

    Harlington Hospice provides a wide range of palliative care services to the residents of Hillingdon and surrounding areas.

    The Hospice hosts a Wellbeing Service (shared with Michael Sobell Hospice), complementary therapies, counselling, lymphoedema service and the CABS team supporting children and adolescents.

    The Hospice also hosts Harlington Hospice@Home team, mainly providing night sits, plus inpatient beds based at Michael Sobell Hospice (MSH) in Northwood.

    Harlington Hospice Bereavement Service

    Harlington Hospice Bereavement Service offers counselling to any adults who have experienced a bereavement.

    Hillingdon Palliative Care Team (hosted by NHS CNWL) provides specialist advice and visiting to palliative care patients living in Hillingdon. Each GP practice has their own named Clinical Nurse Specialist ( CNS ). A Triage CNS manages calls and referrals each day and they have the option to escalate to consultants if needed.

    They can be contacted Monday to Friday 8.00am to 4.30pm excluding bank holidays.

    Hillingdon District Nurses support adult patients who are housebound and need nursing care in their own homes and/ or community settings and who live in Hillingdon or are registered with a Hillingdon GP.

     

     

    Hospice UK can help find a hospice, provide infromation and support about end of life care and guidance to plan ahead.

    Jewish Bereavement Counselling Service

    Jewish Bereavement Counselling Service has counsellors that understand the specific issues raised by bereavement within a Jewish context. This service is sensitive, aware and knowledgeable about social, cultural and religious needs.

    Lullaby Trust

    The Lullaby Trust raises awareness of sudden infant death syndrome (SIDS), provides expert advice on safer sleep for babies and offers emotional support for bereaved families.

    LymphConnect is an online platform developed to help manage lymphoedema or lipoedema, understand more about the condition, share experiences and get support and advice.

    Macmillan provide cancer information, support and help everyone with cancer live life as fully as they can.

    Visit the website to get help with paying bills, advice on benefits or treatment, or to just chat.

    The Centre provides free information and support for anyone affected by cancer:

    • Information on all aspects of living with cancer and treatments
    • Signposting for benefits advice 
    • Enabling access to Macmillan Grants
    • Referrals for wig fittings, headscarves, Look Good Feel Better Programme and Living with Cancer Workshops 
    • Access to Clinical Nurse Specialists

    Marie Curie provide support for patients and carers through terminal illness.

    Other services include Marie Curie nurses, Hospice care, helper and companion services.

    The Michael Sobell Hospice provides a range of palliative services for people with advancing illness.

    The Hospice has 10 beds in its refurbished inpatient unit and looks after people with complex symptoms as well as those close to the end of their life.

    Many people have a short admission and get back home agian.

    The Muslim Bereavement Support Service supports Muslim women who are facing bereavement and/or are bereaved. They will speak with Muslim men and signpost them to appropriate bereavement support.

    Stillbirth and neonatal death

    Stillbirth and neonatal death (Sands) is the leading stillbirth and neonatal death charity in the UK. They ensure that anyone affected by the death of a baby receives the best possible care and support for as long as they need it. 

    Sands provides bereavement support services both nationally through its Freephone helpline, mobile app, online community and resources, and locally through a UK-wide network of around 100 regional support groups. 

    Survivors of Bereavement by Suicide

    Survivors of Bereavement by Suicide are a national charity providing dedicated support to adults who have been bereaved by suicide. The volunteers come from all walks of life but nearly all have been touched by suicide themselves, this experience enables them to connect with others.

    The Compassionate Friends

    The Compassionate Friends (TCF) is a charitable organisation of bereaved parents, siblings and grandparents dedicated to the support and care of other similarly bereaved family members who have suffered the death of a child or children of any age and from any cause.

    Get support using the telephone helpline or online support groups.

    The Good Grief Trust helps all those affected by grief in the UK and brings bereavement services together, to ensure that everyone receives the tailored support they need to move forward with their lives.

    The trust is run by the bereaved, for the bereaved and aim to normalise grief and raise awareness of the impact of grief on a national platform.

    The Wellbeing Hub is an online tool and resource hub developed to empower Hillingdon residents’ and improve overall feelings of wellbeing. 

    It provides modern, hospice wellbeing information alongside other types of wellbeing support. The wellbeing hub empowers people to enhance feelings of wellbeing in a holistic way. The online tool provides support with information to better support each person's own, unique wellbeing journey, by encouraging the setting of personal goals and accomplish tasks that align with individual wellbeing aspirations. Goals such as completing tasks (by creating and sharing a bucket list) and documenting wishes such as an advance care plan can take place.

    Widowed and young (WAY) is a UK charity that offers a peer-to-peer support network for anyone who's lost a partner before their 51st birthday – married or not, with or without children, inclusive of sexual orientation, gender, race and religion.

    Winston's Wish is a national charity offering a free helpline and online support for children and young people wishing to access bereavement support.

    They also provide resources and further training for schools and healthcare professions.

    Y

    Young Minds is a national charity which supports both young people, their parents and loved ones through loss.

    The team is made up of clinical nurse specialists and health care assistants who can support patients with complex needs at home in the last weeks and months of their life (with any diagnosis). 

    The service is open 24 hours a day, 365 days a year, and can offer urgent night visits if needed, but only to patients already referred to the service.
