    The Admiral Nurses at Dementia UK provide information, advice and support with any aspect of dementia.

    The specialist dementia nurses offer free and confidential support via the Helpline for you when you need help.

    Adult Social Care provides practical support to people to stay safe and well.  These services are usually provided in people's homes, care homes or in the community. Thre is also support for people who are caring for an adult family member or friend, access to personal care and home care services.

    Advanced Care Planning in Ealing Care Homes

    This service is for adults with a terminal illness to provide a Universal Care Plan (UCP) to support patients end of life wishes and preferences.

    It is delivered by a Registered Nurse (RN) with comprehensive training and current experience in end of life and palliative care.

    These nurses support the GP Team in creating high quality UCPs for all residents in Care Homes within Ealing borough already needing 24-hour care.

    Age UK provide information and support on financial, legal, health and wellbeing for older people. They support via their advice line, befriending service and day centres.

    Bereavement Advice Centre

    Bereavement Advice Centre provides practical help when you need it most. They support and advise people on what they need to do after a death.

    Bereft Bereavement Support was set up in 1999 to fill the need for specialist bereavement counselling and support for the adult residents (the over 16s) of the London Borough of Ealing.

    Bereavement counselling can help you explore your feelings and come to terms with your grief by talking one to one with a trained counsellor. During these sessions you can talk about how you feel and your counsellor will listen and comment, rather than giving you advice.

    The Bladder and Bowel Service (EBAB) offers expert assessment, consultation and clinical intervention for management of continence care. The teams also provide expert liaison and training to healthcare professionals.

    The support group is a way to learn more about living with a lung condition and share your experiences and stories with others.

    British Heart Foundation

    The British Heart Foundation provides information on heart and circulatory diseases.

    Patients and carers can get support through the online support groups and communities, advice and guidance about heart and circulatory conditions and managing risk factors.

    The British Lymphology Society provides a directory of Lymphoedema treatment services.

    The Ealing Care Co-ordination service offers bespoke support to patients with complex needs who are supported by a variety of services. They can help to align care for patients and support services to come together to aid cohesion in care and improve patient understanding.

    Child Bereavement UK

    Offer a telephone support service for bereaved young people up to age 25, their families/ carers or individuals or families who have lost a child. 

     

    Child Death Helpline

    Child Death Helpline is operated by a dedicated team of volunteers who are all bereaved parents. The helpline provides support to a parent who has lost a child of any age, under any circumstances, and however recently or long ago, we’re here to help. They also offer support to grandparents, siblings and the extended family, as well as professionals who have been impacted by the death of a child.

    Community Palliative Care Team - based at Meadow House Hospice

    The Community Specialist Palliative Care Nurse Team provides advice and visiting to palliative care patients in Ealing and Hounslow. The team are based at Meadow House Hospice.

    The team will visit patients with progressive life limiting illness in their own homes. They provide specialist advice around pain and symptom control and support for patients, their families, and carers during the last stages of illness. They also support patients wishing to die at home through coordination with GPs and hospital teams.

    The team is comprised of medical consultants, specialist nurses, physiotherapists, occupational therapists, social workers, a bereavement support officer and a patient and carer advocacy worker

    The team can be contacted for referrals Monday to Friday 9.00am to 5.00pm.

    Between Monday to Friday 5.00pm and 8.30am, the Out of Hours Telephone Advice line 020 8102 5000 takes calls. Weekends and Bank Holidays 5.00pm to 9.00am.

    Community Team for People with Learning Disabilities Ealing

    The Ealing Community Team for People with Learning Disabilities provides specialist assessment, support and interventions to people with learning disabilities and their families/carers. The team can help to provide expert support to their patients at life’s end and assist with co-ordination and planning across services including liaison with social care and the voluntary/third sector.

    CRUSE Bereavement care

    CRUSE Bereavement care helps people through bereavement with support, information and campaigning.

    D

    The District Nurses are based at Featherstone Road Health Centre, Acton Health Centre and Greenford Green Clinic.

    Referrals can be made Monday to Sunday 8.00am to 8.00pm.

    The Rapid Response District Nurse team are based at Clayponds Hospital and referrals can be made Monday to Sunday 8.00am to 12.00pm.

    F

    There is a legal requirement that a death be registered within five working days. It is important to let the Register Office know if the funeral is to be held within 24 hours. The person registering the death must be able to provide some basic details of the person who has died. Please take the Medical Certificate of Cause of Death to the Register Office. It is also helpful to take the person’s birth and marriage certificates, passport or NHS Medical Card, although they are not essential.

    Grief Encounter is a national charity offering a free helpline and online support for children and young people wishing to access bereavement support.

    IAPT stands for improving access to psychological therapies. IAPT offers talking therapies for people over 18 who have a GP in the London Borough of Ealing.
     
    IAPT can help you with common problems like stress, anxiety and depression and use different talking therapies to help you feel better.

    Jewish Bereavement Counselling Service has counsellors that understand the specific issues raised by bereavement within a Jewish context. This service is sensitive, aware and knowledgeable about social, cultural and religious needs.

    The Lullaby Trust raises awareness of sudden infant death syndrome (SIDS), provides expert advice on safer sleep for babies and offers emotional support for bereaved families.

    LymphConnect is an online platform developed to help manage lymphoedema or lipoedema, understand more about the condition, share experiences and get support and advice.

    Macmillan provide cancer information, support and help everyone with cancer live life as fully as they can.

    Meadow House Hospice provides inpatient palliative care services to the residents of Ealing.

    The hospice has a 15 bedded inpatient unit supported by a team of dedicated staff providing 24/7 specialist support to patients and their family members. Patients with a progressive life limiting illness can be admitted for symptom management or end of life care.

    Patients have a safe, dignified space of their own and the flexibility to spend time alone or in company, surrounded by their possessions and the people they care about. The hospice also has an extensive garden area that provides a quiet and reflective environment for both patients and family members.

    There is a 24 hour telephone support line to the hospice ward on 0208 967 5597.

    Muslim Bereavement Support Service

    The Muslim Bereavement Support Service supports Muslim women who are facing bereavement and/or are bereaved. They will speak with Muslim men and signpost them to appropriate bereavement support. 

    P

    Planned Night End of Life Care

    This is an overnight service and is provided by Marie Curie via Meadow House Hospice for residents of Ealing and Hounslow. This service provides one to one overnight nursing support, symptom control, management of pain, emotional and practical support for patients in their last few weeks of life.

    Referrals are made via Local Clinical Coordination Centre Monday to Sunday from 10.00pm to 7.00am.

    Pumping Marvellous offers support to help people live well with heart failure, to recognise the early symptoms and self-interventions through their community hub.

    R

    Rapid Response End of Life Care Service

    Marie Curie Rapid Response Service provides short term palliative nursing interventions, symptom control, management of pain, advice, emotional and practical support to the residents of Ealing and Hounslow. This service is based at Meadow House Hospice.

    The service can be contacted Monday to Sunday from 6.00pm to 7.00am.

    Rapid Response Referral Form (DOCX)

    The Ealing Rapid Response Service provides admission avoidance in the community which supports expert clinical assessment and interventions to patients requiring urgent care and assessment at home. The service does not initiate planned interventions and should not be used in advance of other service like District Nursing and Marie Curie to provide care to patients in receipt of palliation.  The team can offer assessment and interventions where there is an urgent need for clinical support outside of the operating times for planned services which includes out of hours’ symptom management and assessment.

    Patients can be referred to the service via the GP, 111 or by communicating with the Ealing Community Partners (ECP) Referrals Hub via telephone.

    Samaritans provides a 24 hour, 7 days a week advice line.

    Stillbirth and neonatal death

    Stillbirth and neonatal death (Sands) is the leading stillbirth and neonatal death charity in the UK. They ensure that anyone affected by the death of a baby receives the best possible care and support for as long as they need it. 

    Sands provides bereavement support services both nationally through its Freephone helpline, mobile app, online community and resources, and locally through a UK-wide network of around 100 regional support groups. 

    Survivors of Bereavement by Suicide

    Survivors of Bereavement by Suicide are a national charity providing dedicated support to adults who have been bereaved by suicide. The volunteers come from all walks of life but nearly all have been touched by suicide themselves, this experience enables them to connect with others.

    The Compassionate Friends

    The Compassionate Friends (TCF) is a charitable organisation of bereaved parents, siblings and grandparents dedicated to the support and care of other similarly bereaved family members who have suffered the death of a child or children of any age and from any cause.

    The Good Grief Trust helps all those affected by grief in the UK and brings bereavement services together, to ensure that everyone receives the tailored support they need to move forward with their lives.

    The trust is run by the bereaved, for the bereaved and aim to normalise grief and raise awareness of the impact of grief on a national platform.
