The Admiral Nurses at Dementia UK provide information, advice and support with any aspect of dementia.

The specialist dementia nurses offer free and confidential support via the Helpline for you when you need help.

Adult Social Care provides practical support to people to stay safe and well.  These services are usually provided in people's homes, care homes or in the community. Thre is also support for people who are caring for an adult family member or friend, access to personal care and home care services.

Age UK provide information and support on financial, legal, health and wellbeing for older people. They support via their advice line, befriending service and day centres.

The Alzheimer's Society dementia advisors can help people living with dementia and their carers. They can provide practical help and emotional support via the telephone support or the online community connecting those in similar situations.

Asthma + Lung UK provides help for those living with lung conditions.

There is support available using the telephone helpline or the email address above.

Bereavement Advice Centre provides practical help when you need it most. They support and advise people on what they need to do after a death.

The Bereavement Support Team offers confidential, individual support to help cope with the loss of family or loved ones. The support is available to anyone who has experienced the death or affected by the death of a loved one  who was cared for by St Lukes. 

The support group is a way to learn more about living with a lung condition and share your experiences and stories with others.

BBS has a team of highly skilled, professional and supervised volunteer counsellors who reflect the many different cultures of the population of Brent, and speak a variety of languages.

They provide counselling, advice, information & training for bereaved people, their families, friends and our community in the London Borough of Brent.

Brent Carers Centre deliver a whole range of support services to address the social, emotional and financial issues that carers experience.

The service operates between Monday to Friday 8:30am to 5.00pm.

Referrals for same/next day appointments need to be received by 4.30pm Monday to Friday.

Brent North - Chalkhill team based at Chalkhill Primary Care Centre and Wembley team at Wembley House

Brent South - Harlesden and South Kilburn teams based at Hillside Primary Care Centre and Willesden team at Willesden Centre for Health

All referrals should be sent to the Brent DN SPA email (Single Point of Access) and should include contact telephone numbers.

North Brent Community Specialist Palliative Care Nurse team provides advice and visiting to palliative care patients in North Brent. The team is based at St Lukes Hospice.

Areas: Queensbury, Fryent, Welsh Harp, Barnhill, Kenton, Northwick Park, Preston, Sudbury, Wembley Central, Alperton, Tokyngton

The team can be contacted for referrals Monday to Sunday 8.30am to 4.30pm. 

After 4.30pm the Out of Hours Telephone Advice line 020 8382 8000 operates and calls will be taken by a nurse on St Lukes inpatient unit

South Brent Community Specialist Palliative Care Nurse Team provides advice and visiting to palliative care patients in South Brent. The team are based at Pembridge Hospice

Areas: Harlesden, Willesden, Kilburn, Neasden (south of north circular Brent, except the St Raphael's Estate) 

The team can be contacted for referrals Monday to Friday 8.30am to 5.00pm. Weekends and Bank Holidays 9.00am to 5.00pm.  

Between Monday to Friday 5.00pm and 8.30am, the Out of Hours Telephone Advice line 020 8102 5000 takes calls. Weekends and Bank Holidays 5.00pm to 9.00am.

Offer a telephone support service for bereaved young people up to age 25, their families/ carers or individuals or families who have lost a child. 

Child Death Helpline is operated by a dedicated team of volunteers who are all bereaved parents. The helpline provides support to a parent who has lost a child of any age, under any circumstances, and however recently or long ago, we’re here to help. They also offer support to grandparents, siblings and the extended family, as well as professionals who have been impacted by the death of a child.

CNWL Talking Therapies Service provides psychological therapies. Self-referral accepted via the website for adults who are registered with a Brent, Harrow or Hillingdon GP.

Support is available for a range of difficulties, such as low mood or depression, anxiety, and stress, phobias, physical health anxiety, PTSD and Insomnia.

CRUSE Bereavement care helps people through bereavement with support, information and campaigning.

This overnight service is for the Harlesden Branch Matron.

The twilight hours are Monday to Sunday 5.00pm to 12.00am. The night shift hours are Monday to Sunday 12.00am to 8.30am.

This service provides a quick funding route for HCA packages of care for appropriate patients in Brent and Harrow for 2 weeks whilst Continuing Health Care (CHC) is arranged.

Funding is to pay for social care at home or a place in a care home for patients with a deteriorating condition and short prognosis 

Referrals by clinicians can be made Monday to Saturday 9.00am to 4.00pm and Sunday between 8.30am to 8.30pm. Referrals made after 4pm will be dealt with the next working day.

On-call advice will be given until 8.30pm for known patients.

There is a legal requirement that a death be registered within five working days. It is important to let the Register Office know if the funeral is to be held within 24 hours. The person registering the death must be able to provide some basic details of the person who has died. Please take the Medical Certificate of Cause of Death to the Register Office. It is also helpful to take the person’s birth and marriage certificates, passport or NHS Medical Card, although they are not essential.

Jewish Bereavement Counselling Service has counsellors that understand the specific issues raised by bereavement within a Jewish context. This service is sensitive, aware and knowledgeable about social, cultural and religious needs.

The Muslim Bereavement Support Service supports Muslim women who are facing bereavement and/or are bereaved. They will speak with Muslim men and signpost them to appropriate bereavement support.

Stillbirth and neonatal death (Sands) is the leading stillbirth and neonatal death charity in the UK. They ensure that anyone affected by the death of a baby receives the best possible care and support for as long as they need it. 

Sands provides bereavement support services both nationally through its Freephone helpline, mobile app, online community and resources, and locally through a UK-wide network of around 100 regional support groups.

Survivors of Bereavement by Suicide are a national charity providing dedicated support to adults who have been bereaved by suicide. The volunteers come from all walks of life but nearly all have been touched by suicide themselves, this experience enables them to connect with others.

The Compassionate Friends (TCF) is a charitable organisation of bereaved parents, siblings and grandparents dedicated to the support and care of other similarly bereaved family members who have suffered the death of a child or children of any age and from any cause.

The Good Grief Trust helps all those affected by grief in the UK and brings bereavement services together, to ensure that everyone receives the tailored support they need to move forward with their lives.
