The Adult Assessment Unit (AAU) hub at Barnet Hospital provides rapid access to multi-specialty and medical ambulatory pathways for acute and elderly care patients.

Referral into the service will be via the AAU Hub navigator, this is for healthcare professionals only.

Referring to out of hours,  Monday to Friday 8.00am to 8.00pm and Saturday to Sunday 8.00am to 6.00pm, phone the switchboard, 020 8216 4600 and ask for the medical registrar.

The Camden rapid access service is for adults over 18 years of age living Camden or registered with a Camden GP and living within one mile border of Camden.

This service is available every day from 8.00am to 10.00pm including weekends and bank holidays.

Hillingdon Rapid Response Service provides a single point of contact for adult patients experiencing a health crisis who could be safely cared for in the community instead of being admitted to hospital.

This service is available every day from 9am to 12.30am.

The Paediatric Rapid Access Clinic offers consultant-led urgent referral appointments for children.

The rapid care and response teams manage a range of conditions including acute exacerbations and acute complications of long-term conditions in the community.

The service is available every day between 8.00am and 8.00pm.

The Brent Rapid Response Team provides admission avoidance in the community which supports expert clinical assessment and interventions to patients requiring urgent care and assessment.

Office hours are Monday to Sunday 8.00am to 10.30pm.

The Ealing Rapid Response Service provides admission avoidance in the community which supports expert clinical assessment and interventions to patients requiring urgent care and assessment at home. The service does not initiate planned interventions and should not be used in advance of other service like District Nursing and Marie Curie to provide care to patients in receipt of palliation.  The team can offer assessment and interventions where there is an urgent need for clinical support outside of the operating times for planned services which includes out of hours’ symptom management and assessment.

Patients can be referred to the service via the GP, 111 or by communicating with the Ealing Community Partners (ECP) Referrals Hub via telephone.

Harrow rapid response is a prevention of admission service that visits patients at home, to assess and provide treatment to prevent an unnecessary admission to hospital.
