Fatigue: Information for clinicians

Fatigue is an overwhelming symptom that affects quality of life. It is a very common symptom in advancing illness but a structured approach to assessment and treatment can help.

Key clinical features for a clinician to assess in the community

Ask how the fatigue affects daily life and what the patient would be doing if they had more energy. Help them to consider practical solutions.

Exclude reversible causes including:

  • Medication side effects like opiates, some antiemetics (e.g. haloperidol), neuropathic agents (e.g. gabapentin, pregabalin), anxiolytics (e.g. benzodiazepines), mirtazapine. Consider dose reduction.
  • Anaemia and electrolyte disturbances
  • Pain or other symptoms interfering with sleep
  • Anxiety or depression
  • Poor nutrition or dehydration
  • Deconditioning with muscle loss

However in the final weeks of a long term illness fatigue can be very severe and hard to improve. Fatigue often has more than one cause, including the underlying disease process.

Initial tests and treatment
  • Full blood screen for reversible causes, e.g. FBC, TFT, U&E, B12/folate, LFTs, haematinics, Ca2+ and magnesium
  • Realistic advice about nutrition, sleep and exercise - please see Tiredness: Information for patients and carers
  • Mood review and medication review - can sedative medications be reduced?
Specialist care
  • Steroids such as dexamethasone or prednisolone can really improve appetite and energy levels. Although the impact is often short lasting, it can be very helpful for important events or days out. Common practice is to start with dexamethasone 4mg once a day early in the morning (so it doesn't interfere with sleep) and continue with the lowest dose that still gives benefit
  • Psychostimulants occasionally may help, e.g. methylphenidate, if no cardiac contraindications but the evidence base is weak
Top Tips
  • If using steroids as a boost for a special occasion start the day before and give early in the morning (so not to interfere with sleep). A course of less than 2 weeks duration doesn't require tapering
  • Consider a B12 trial: 6 × 1 mg hydroxocobalamin subcutaneously over 2 weeks may improve fatigue. Continue based on symptomatic response. Higher-risk groups include those with poor nutrition, excess alcohol or a history of B12 deficiency

The team is made up of clinical nurse specialists and health care assistants (HCA) who can support patients with complex needs at home in the last weeks and months of their life (with any diagnosis). 

The service is open 24 hours a day, 365 days a year, and can offer advice.

 Urgent night visits  are available to patients already referred to the service.

If you have already been referred to this service, you will have been given a direct contact number.

Speak to your GP or district nurse to be referred to the service.

The team can be contacted for referrals Monday to Sunday 8.30am to 4.30pm. 

After 4.30pm the Out of Hours Telephone Advice line 020 8382 8000 operates and calls will be taken by a nurse on St Lukes inpatient unit

South Brent Community Specialist Palliative Care Nurse Team provides advice and visiting to palliative care patients in South Brent. The team are based at Pembridge Hospice

Areas: Harlesden, Willesden, Kilburn, Neasden (south of north circular Brent, except the St Raphael's Estate) 

All referrals to be made via the CLCH Single Point of Access Service (SPA) email address and will be processed during working hours of Monday to Friday 9.00am to 4:30pm. Any referrals sent outside of this time period will be processed on the next working day.

Community Palliative Care Team - based at Meadow House Hospice

The Community Specialist Palliative Care Nurse Team provides advice and visiting to palliative care patients in Ealing and Hounslow. The team are based at Meadow House Hospice.

The team will visit patients with progressive life limiting illness in their own homes. They provide specialist advice around pain and symptom control and support for patients, their families, and carers during the last stages of illness. They also support patients wishing to die at home through coordination with GPs and hospital teams.

The team is comprised of medical consultants, specialist nurses, physiotherapists, occupational therapists, social workers, a bereavement support officer and a patient and carer advocacy worker

The team can be contacted for referrals Monday to Friday 9.00am to 5.00pm.

Between Monday to Friday 5.00pm and 8.30am, the Out of Hours Telephone Advice line 020 8102 5000 takes calls. Weekends and Bank Holidays 5.00pm to 9.00am.

Hillingdon Palliative Care Team (hosted by NHS CNWL) provides specialist advice and visiting to palliative care patients living in Hillingdon. Each GP practice has their own named Clinical Nurse Specialist ( CNS ). A Triage CNS manages calls and referrals each day and they have the option to escalate to consultants if needed.

They can be contacted Monday to Friday 8.00am to 4.30pm excluding bank holidays.

St John's Hospice Inpatient unit

St. John's Hospice provides inpatient palliative care services to the residents of Brent, Central London and West London.

This is an inpatient unit with 18 beds

Please contact for admissions for advice and Community Specialist Palliative Care.

The office hours are Monday to Friday 8.30am to 4.30pm. Referrals received in the morning are triaged the same afternoon, and those received in the afternoon are triaged the following working day.

If urgent, please follow up the referral by calling us on 020 7806 4040. Out of hours, urgent referrals will be discussed with the consultant on call.
