    The team is made up of clinical nurse specialists and health care assistants (HCA) who can support patients with complex needs at home in the last weeks and months of their life (with any diagnosis). 

    The service is open 24 hours a day, 365 days a year, and can offer advice.

     Urgent night visits  are available to patients already referred to the service.

    If you have already been referred to this service, you will have been given a direct contact number.

    Speak to your GP or district nurse to be referred to the service.

    Carers Network is an independent local charity reaching over 5,300 unpaid carers in the most isolated and deprived pockets of the City of Westminster, the London Borough of Hammersmith and Fulham and the Royal Borough of Kensington and Chelsea. We have been proudly supporting carers in central London since 1991.

    There is help for new carers and a carer's assessment to discuss how to maintain your health and balance your caring responsibilities with other areas of your life. It may also help you get financial support.

    Carers Trust works to transform the lives of unpaid carers through its network of local carer organisations. Carers can access to support, advice and resources to help them manage their role as carers.

    Carers Trust Hillingdon offer support and guidance to unpaid carers of all ages throughout the London Borough of Hillingdon.

    They provide information, advice, training and support and a range of Fact Sheets on their website.

    Calls will be routed directly to the clinical teams that operate after 5.00pm.

    District Nurses Central London and West London

    District Nurse referrals can be made Monday to Friday 8.00am to 5.00pm.

    The District Nurses are based at Featherstone Road Health Centre, Acton Health Centre and Greenford Green Clinic.

    Referrals can be made Monday to Sunday 8.00am to 8.00pm.

    The Rapid Response District Nurse team are based at Clayponds Hospital and referrals can be made Monday to Sunday 8.00am to 12.00pm.

    District Nurses Hammersmith and Fulham

    District Nurse referrals can be made Monday to Sunday 9.00am to 5.00pm.

    Harrow District Nursing Service (CLCH) provide routine monitoring tasks and syringe drivers Monday to Friday 8.00am to 6.00pm and on Saturdays and Bank Holidays 8.00am to 4.00pm. Please select option 3 from the call list.

    The Rapid Response team provide (7/7) For prn sc medication and referrals can be made Monday to Friday 8.00am to 6.00pm. Please select option 2 from the call list.

    Hillingdon District Nurses support adult patients who are housebound and need nursing care in their own homes and/ or community settings and who live in Hillingdon or are registered with a Hillingdon GP.

    The Ealing Carers Service support carers who have caring responsibilities living within the borough of Ealing.

    Ealing Carers Partnership offers support services to unpaid carers aged 18 and over, living and caring in the London Borough of Ealing.

    Harrow Carers is an independent charity and voluntary organisation and has been the lead carers’ support agency in Harrow since 1996. Support and services are available to anyone who cares for people in the community who are ill, frail, disabled and disadvantaged.

    Planned Night End of Life Care

    This is an overnight service and is provided by Marie Curie via Meadow House Hospice for residents of Ealing and Hounslow. This service provides one to one overnight nursing support, symptom control, management of pain, emotional and practical support for patients in their last few weeks of life.

    Referrals are made via Local Clinical Coordination Centre Monday to Sunday from 10.00pm to 7.00am.

    St Luke's Hospice at Home

    St. Lukes Hospice@Home provides palliative care services to the residents of North Brent and Harrow. Respite and bridging packages of care are available for end of life patients.

    Marie Curie Night Nurses may also be booked by the Hospice at Home Team
