Continuing Healthcare (CHC)
The West and North Integrated Care Board (WNL ICB) NHS Continuing Healthcare (CHC) is a package of care arranged and funded by the NHS for adults aged 18 and over who have significant ongoing health needs.
It is provided outside of hospital and is available to people who have been assessed as having a primary health need. This may include people with complex long-term conditions, disabilities, serious illness or those nearing the end of life.
If someone is eligible, the WNL ICB will fund the package of health and social care
