Understanding advanced heart failure: Information for patients and carers

What Does Advanced Heart Failure Mean?

Heart failure means that the heart isn't pumping blood around the body as well as it should and oxgyen levels may drop and not reach tissues such as lungs and muscles.

When the heart becomes very weak and medication is not controlling symptoms this is called advanced heart failure. At this stage medication should be reviewed regularly to keep symptoms as well controlled as possible.

Care homes assessment team (CHAT) is a nurse-led service offering physical and mental health support to residents in care homes across Enfield.

The team enable residents to remain in their care home and provide a visiting or telephone service depending on an individual’s needs to diagnose and treat complex physical and mental health including palliative and end of life care support in a resident’s care home.

The palliative care team is multidisciplinary and works in North Middlesex University Hospital to provide:

  • advice and support on symptom control related to pain, nausea and vomiting, breathlessness, and fatigue
  • psychological support 
  • support and advice on social and financial issues
  • advanced care planning
  • discharge planning
  • end-of-life care
  • bereavement support

The palliative care team is available every day from 9.00am to 5.00pm.
